Is the crankcase ventilation hose the rubber T-shaped hose on top of the engine ?

No, that is the vacuum feed to the booster and other vacuum modules. The crankcase INTAKE ventilation hose is on the passenger side valve cover and connects to the intake tube. On the drivers side valve cover there is a PVC hose/tubing that connects to the PVC valve and the intake manifold.
The crankcase is ventilated from the passenger side valve cover through the crankcase and out the PCV valve on the drivers side valve cover into the intake using metered air from the intake tube.

I posted ^ up there ^ that I have a bunch of them brand new, and would be willing to sell you one. You can use regular bulk vacuum line from the parts store for the rest of it.

Thanks for the offer. I checked the Tee and it appears in good condition.
If the hoses can be replaced with bulk vacuum hose, I'll start with that.
When replacing those vacuum hoses, is there a special procedure or can they just be pulled off and replaced ?

Let's back up a minute... what is it you're trying to accomplish? Are you getting a code or something? A vacuum leak?

No code.
While replacing the coolant system hoses, I noticed the hoses appeared slightly cracked and somewhat crunchy looking.
Wouldn't be surprised if they were the original hoses.
And since bulk hose doesn't cost much, it seemed a good idea to replace them before they fail.

Just pull them off, cut new ones to fit, and replace.
