Is the crankcase ventilation hose the rubber T-shaped hose on top of the engine ?
No, that is the vacuum feed to the booster and other vacuum modules. The crankcase INTAKE ventilation hose is on the passenger side valve cover and connects to the intake tube. On the drivers side valve cover there is a PVC hose/tubing that connects to the PVC valve and the intake manifold.
The crankcase is ventilated from the passenger side valve cover through the crankcase and out the PCV valve on the drivers side valve cover into the intake using metered air from the intake tube.

What would be the part number for the T-shaped vacuum feed to the booster ?
Is that the brake power booster ?

It's part number LZ-6C324-AA.
Now it's just a matter of finding a new one.
I posted ^ up there ^ that I have a bunch of them brand new, and would be willing to sell you one. You can use regular bulk vacuum line from the parts store for the rest of it.
Let's back up a minute... what is it you're trying to accomplish? Are you getting a code or something? A vacuum leak?
