Strange problem HELP!

When i ever i have to aggressively break, my transmission will basically pop out of gear. Then to get going again i have to rev it up a bit, then it will slam back into gear very rough and be perfectly fine after that. For example, ill be going about 40 mph. Stop sign ahead. Ill break late and come to a complete stop. When i go to take off, it just revs up as if its in neutral and will kick into gear after i give it enough gas. I have had my tranny rebuilt and have recently upgraded to drilled/slotted rotors and ceramic pads. In addition, no codes are thrown when this happens. To deal with the jerkiness of the slamming into gear, ill keep my left foot on the brake and rev it with the right when i notice it has"slipped" out. This prevents the embarrassing starts from stops but im tired of the issue. Anyone have any ideas on what this could be?
 
Bet ya that your transmission fluid level is low.
