Phone Clip

I saw this picture on the homepage of LVC today, I was going to send him a message but he hasn't been on since 2005 so...

Does anyone know if that is a readily available clip for order, and where I can order one. Or is it custom, in which case, it looks really clean. I've never seen one on here before and it's exactly what I want.
